HEALTH CARE PROXY

 Who is Your Health Care Agent? Every competent adult, 18 years old and older, has the right to appoint a Health Care Agent in a Health Care Proxy. The Department of Public Health is partnering with Honoring Choices Massachusetts and the Massachusetts Health & Hospital Association to offer a simple way to choose a Health Care Agent and complete a Health Care Proxy.
